Reporting an Incident

Please use the below form when reporting any emergency involving participants or any situations that impact their health, safety, or welfare.

Employers, partner agents and third parties with whom ERDT has current agreements are required to report incidents within 1 business day, however we encourage anyone to use this form to report an incident.

The information you provide will be transmitted directly to an ERDT Work & Travel Program staff member and will be addressed more quickly and efficiently than a phone call. Thank you.
